advocate dedicated to the “YourChoice” plan. Call the
pharmacist at 941.748.4501, ext. 6406. Meet the
YourChoice Pharmacy Advocate.

The pharmacy plan is administered by Prescription
Solutions. They have a customer service center and can
answer all types of questions about current and past
prescriptions. Call them at 800.797.9791.

Preferred Retail And Mail Order Pharmacies
These retail pharmacies offer the lowest copay for
members.
  • No co-pay for generics (free of charge to the
    member)
  • Minimum of $10 copay for preferred brand
    medications or 20% of the approved cost
  • Minimum of $30 copay for non-preferred brand
    medications or 40% of the approved cost
